Local search is not one ranking - it is a different result for every corner of your catchment
Most clinic owners think about local SEO as a single position: are we first, or are we not? That framing is why the reporting never quite matches what you see on your own phone.
Local results are assembled per search, and one of the biggest inputs is where the person is standing when they type. Two people searching the same words from opposite ends of your suburb can be shown a different set of clinics. There is no one ranking to hold.
What that means in practice
Your visibility is better understood as coverage across an area, not a number. A clinic can sit near the top for searches made within a few streets of the front door and be well down the list for the same search made a fifteen minute drive away - in the shopping centre, the office district, the newer estate on the edge of town where the people you want are actually standing.
That gap is not a fault. It is how local results work. But it does change what is worth doing about it.
The part owners can act on
Proximity is the input you cannot change. The two you can influence are what your listing and your site actually say, and how established they are for the specific thing being searched.
Most clinics describe themselves broadly - "skin clinic", "cosmetic clinic" - and then wonder why they do not appear for the individual treatment someone typed. Named services, written out plainly and supported by pages that cover them properly, are what let you show up further from the door for that search. Broad wording only competes close to home.

Before you look at a report again
Ask which part of your catchment the numbers describe. A single average position across an area tells you almost nothing about whether you are visible where your patients are. Coverage is the honest question.
